1. Introduction

Welcome to screenings4u! These Terms of Service govern your use of our Corporate Packages. By accessing or utilizing our services, you agree to adhere to these terms in full.

2. Services Offered

screenings4u provides a wide range of services within our Corporate Packages, including but not limited to:

Background Checks (fees may vary based on location)
Random Drug and Alcohol Pool Management
Drug & Alcohol Policy Development
Designated Employer Representative (DER) Training
Drug-Free Workplace Training (for employees)

Bronze Package

The screenings4u Bronze Package includes services for up to 25 employees or contractors:

10 Panel NDOT or 5 Panel DOT Drug Test

Silver Package

The screenings4u Silver Package covers up to 50 employees or contractors:

Gold Package

The screenings4u Gold Package includes services for up to 100 employees or contractors:

10 Panel NON DOT Drug Tests include:

DOT 5 Panel Drug Tests include:

Marijuana (THC)

4. Payment Terms

To initiate any Corporate Package, a down payment is required. The remaining balance must be settled within 30 days of the initial payment. Clients will receive a comprehensive cost breakdown before finalizing any agreement.

5. Additional Fees

Fees for background checks may vary by geographic location. screenings4u will inform clients of any potential additional fees before service delivery for complete transparency.

6. Client Responsibilities

Clients must provide accurate and complete information regarding employees or contractors being tested. It is the client’s responsibility to comply with all relevant laws and to obtain necessary consent before testing.

7. Confidentiality

All client information and testing data will be handled with strict confidentiality, in compliance with applicable laws and regulations. Results will only be shared with authorized individuals designated by the client.

8. Limitation of Liability

screenings4u shall not be liable for any indirect, incidental, or consequential damages resulting from the services provided. Our maximum liability for any claims related to our services will not exceed the fees paid for the specific service in question.

9. Amendments

screenings4u reserves the right to modify these Terms of Service at any time. Clients will be notified of any significant changes via email or notifications in our online portal.

10. Cooperation with HR Teams

screenings4u will work alongside your HR team to ensure effective implementation and execution of our services.

11. Location Restrictions

All screenings4u packages are intended for employees visiting our Chicago, IL, location. For on-site services, please submit an on-site request form.

12. Commitment

A one-year commitment is required upon purchasing any package, with services commencing on the date of purchase. Random testing will occur quarterly throughout the year.

13. On-site Testing

On-site testing is available as an add-on. Interested customers must complete the on-site request form available on our website.

14. Breathalyzer Testing

Breathalyzer testing can be added for an additional $25 per employee.

15. Governing Law

These Terms of Service will be interpreted in accordance with the laws of the State of Illinois.
