1. Acceptance of these terms
By visiting a screenings4u website, creating an account, submitting an order, or using a service, you agree to these Terms of Use and the policies incorporated by reference. If you use the services for an organization, you represent that you have authority to bind that organization.
If you do not agree, do not use the websites or services.
2. Services and eligibility
screenings4u provides or coordinates drug and alcohol testing, specimen collection, background-screening support, DOT and workplace compliance services, training, consulting, and related account tools. Service availability, collection locations, turnaround times, and requirements vary by jurisdiction and provider.
You must be at least 18 years old and legally capable of entering a contract. Certain services require identity verification, donor consent, employer authorization, prescriptions, chain-of-custody documentation, or other information.
3. Accounts and security
- Provide accurate, current, and complete information.
- Keep credentials confidential and use only accounts you are authorized to access.
- Notify us promptly of suspected unauthorized activity.
- Do not share results or personal information except as permitted by law and applicable authorization.
You are responsible for activity conducted through your account. We may suspend access when reasonably necessary to protect users, data, or our systems.
4. Orders, pricing, and payment
Prices, taxes, third-party fees, and service details are shown during checkout or in an accepted proposal. You authorize the selected payment method for all stated charges. An order is subject to verification and acceptance.
Scheduling, lab processing, digital access, shipment, or other fulfillment may begin after payment. Cancellations and refunds are governed by our Refund Policy. Payment processing may be provided by third parties whose terms also apply.
5. Acceptable use and customer responsibilities
You may not misuse the services, interfere with security, upload malicious code, impersonate another person, scrape or resell content without permission, or use results unlawfully. Employers and regulated parties remain responsible for determining which laws, regulations, policies, notices, consents, and testing procedures apply to them.
Nothing on the website replaces advice from legal counsel, a medical professional, a Medical Review Officer, or the relevant regulatory agency.
6. Intellectual property
The websites, software, branding, designs, text, training materials, and other content are owned by or licensed to screenings4u and protected by applicable law. We grant you a limited, revocable, non-transferable right to use them for their intended purpose. No ownership rights are transferred.
7. Third parties and disclaimers
Laboratories, collection sites, payment providers, carriers, identity providers, and other independent third parties may participate in fulfillment. We do not control every third-party system or its availability.
To the maximum extent permitted by law, websites and informational materials are provided “as is” and “as available.” We do not guarantee uninterrupted access, a specific result, regulatory outcome, or turnaround time. Rights that cannot lawfully be disclaimed remain unaffected.
8. Limitation of liability and indemnity
To the maximum extent permitted by law, screenings4u will not be liable for indirect, incidental, special, consequential, exemplary, or punitive damages, or loss of profits, data, goodwill, or business opportunity arising from use of the services. Any aggregate liability will not exceed the amount paid for the service giving rise to the claim during the preceding twelve months.
You agree to indemnify screenings4u from third-party claims arising from your unlawful use, breach of these terms, inaccurate instructions, or violation of another person’s rights, except to the extent prohibited by law.
9. Governing law, changes, and severability
These terms are governed by the laws of Illinois, without regard to conflict-of-law principles, unless applicable law requires otherwise. Courts located in Cook County, Illinois will have exclusive jurisdiction unless the parties agree to another lawful dispute process.
We may update these terms prospectively. The effective date identifies the latest version. If a provision is unenforceable, the remaining provisions continue in effect. Failure to enforce a provision is not a waiver.
